Health Forms & Swim Checks

Overview

BSA requires current health forms for all participants in scouting activities. Scout Manager tracks health form status and swim check classifications so you never arrive at camp with expired paperwork.

Required role: Administrator or Leader

Health Form Dashboard

Navigate to Membership → Health Forms to see the dashboard. It shows:

  • Current — members with valid, non-expired health forms
  • Expiring Soon — forms expiring within 30 days
  • Expired — forms that have passed their expiration date
  • Missing — members with no health form on file

Recording Health Form Dates

From the Dashboard

  1. Go to Membership → Health Forms
  2. Click on a member’s name
  3. Enter or update the expiration dates for:
    • Part A/B — required for all activities
    • Part C — required for events over 72 hours
    • Part D — conditional (diving, etc.)
  4. Click Save

From a Member’s Profile

  1. Open any scout or adult’s profile
  2. Scroll to the Health Form section
  3. Edit the expiration dates inline

Swim Check Classifications

BSA swim classifications determine water activity access:

ClassificationDescription
Non-swimmerCannot pass the basic swim test
BeginnerPasses beginner test (jump in, swim 25 feet, rest 1 minute)
SwimmerPasses full swimmer test (jump in, swim 75 yards, rest, tread water)

Recording Swim Checks

  1. Go to Membership → Health Forms
  2. Click on a member
  3. Select their swim classification from the dropdown
  4. Enter the test date
  5. Click Save

Tips

  • Health form expiration reminders can be included in personalized newsletters
  • Filter the dashboard by den/patrol to check a specific group before a campout
  • Print the dashboard as a compliance report for your records
